Columbus County Health Department is proud to announce a new service at your local health department. We are now able to provide the Commercial Driver’s License physical examination, better known as “CDL physical exam”. Dr. Susan Aycock is now certified to complete the physical exam. All components of the exam can be completed at your local health department.
If you or someone you know needs the CDL physical exam, please call 910-640-6615 to schedule an appointment. The fee is $130.00. All insurances gladly accepted.

We accept the following insurance, Blue Cross Blue Shield, Blue Medicare, Champ VA, Cigna, Humana, Medcost, Medicaid, Medicare, NC Health Choice, NC State Health Plan, Tricare, Railroad Retirement, UMR, United Healthcare, and self-pay. The Department has sliding fee scale for 200% above poverty level to assist those with a need.
