CAP CASE MANAGER
General Statement of Duties
Plans, coordinates, and implements In-Home Adult Care, CAP/D, Respite, and 3B service programs for the senior citizens in the County.
Distinguishing Features of the Class
An employee in this class is responsible for assessing, evaluating and coordinating with the Senior Citizen Nurse to determine if an individual in nursing home level of physical care can be maintained safely in their home along with physical and mental support of family members. The care must comply with CAP/DA rules and regulations and maintain the health, safety and well being of Community Alternative Program/DA participants in their caseload. Employee must assure level of care stays within the client’s budget and the employee must manage, monitor and coordinate Community Alternative Program/DA services monthly. Case management duties include processing referrals for new clients, coordinating with local providers to obtain needed approvals and information to assist the client, developing plans of care, monitoring services, and providing case management services daily. Employee ensures consistency in all operational components of the program to include client evaluations. Employee handles vendor issues, and works with the local Department of Social Services on eligibility issues. Employee exercises independence in initiating and executing case management activities within budgetary limitations. Work rules and policies are subject to change as a result of federal and state program modifications. Work is performed under the general supervision of the Senior Citizen Program Supervisor and is evaluated by the effectiveness and quality of provided services.
Duties and Responsibilities
Essential Duties and Tasks
Monitors, coordinates and assist Community Alternative Program/DA clients in the home to ensure their health, safety and well-being.
Requests Fl-20s from the Senior Citizen Nurse to ensure eligibility and signatures; completes assessment with client and caregiver; verifies Medicare and Medicaid coverage.
Verifies all services rendered to the client such as Senior Citizen Nurse visits, durable medical equipment, diabetic testing supplies, etc.
Reviews and coordinates with the RN her assessment of the client’s physical condition and what ADLs and IADLs the client is able to perform independently.
Reviews and verifies family support as required by CAP/DA.
Determines the type and amount of assistance required to keep the client at home in a safe environment and prevent the client from being admitted to a facility of care; develops a Plan of Care.
Sends approval notifications to all providers once DSS approves notice of benefits.
Contacts the client monthly via telephone or a home visit to assess the client; notify service providers.
Verifies billing information from Medicaid and makes any modifications if needed; prepares in house billing for in home services.
/conducts Continued Needs Review Assessments and prepares paperwork for agency and provider to continue services.
Conducts Change of Status: re-admits for clients in crisis/ substantial changes.
Coordinates and maintains a good working relationship with service providers and health care entities to obtain needed approval and information to coordinate services for clients.
Additional Job Duties
Review and approve plans of care.
Serves as the Senior Citizen Program Supervisor in her absence.
Provides front desk coverage as needed.
Performs related duties as required.
Recruitment and Selection Guidelines
Knowledges Skills and Abilities
Working knowledge of basic social work principles, techniques, and practices and their application to the casework.
Considerable programmatic knowledge of Community Alternative Programs, Respite Care and In Home Care Adult programs.
Knowledge of the principles, techniques and practices and their application to specific casework problems related to Adult In Home programs.
Knowledge of federal, state, and local resources to aid clients.
Knowledge of socioeconomic issues affecting case management.
Knowledge of the laws, regulations and policies governing the provided services.
Ability to express ideas clearly and concisely and to plan and execute work effectively
Ability to establish and maintain effective working relationships with other employees, community and state agencies, and the general public.
Physical Requirements
Must be able to physically perform the basic life operational functions of stooping, kneeling, reaching, standing, walking, fingering, talking, and hearing.
Must be able to perform light work exerting 20 pounds of force occasionally; and 10 pounds of force frequently.
Must possess the visual acuity to prepare and analyze data and figures, operate a computer terminal, and determine the accuracy, neatness, and thoroughness of work assigned.
Desirable Education and Experience
Graduation from a four year university or college with a degree in the social work or other social science areas; or an equivalent combination of education and experience.
Special Requirement
Possession of a valid North Carolina driver's license.
