Social Worker II/Grade 67

Salary:       Hiring $40,167  Min $42,175 to Max $60,251

Description-Social Worker or Registered Nurse will provide comprehensive Case Management services to pregnant women with conditions or factors that could have a negative impact on the pregnancy outcome.  The primary focus of this position is to ensure that the patient is linked with all appropriate services for medical care, behavioral health, and community resources and to provide a core set of educational interventions at established timelines.  Patient contact is based on the level of need and includes clinic visits, hospital visits, contact by phone, and some home visits.  The Case Manager serves as liaison between patient and service providers and works to ensure a healthy outcome for Mother and child.

Minimum Education and Experience: 

Social Worker:  Bachelor’s Degree in social work (BSW, BA in SW, or BS in SW) or Master’s Degree in social work (MSW, MA in SW, or MS in SW) from a Council on Social Work Education accredited social work degree program. 

Registered Nurse: Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation, or Master’s in Public Health graduation from a school of professional nursing; or graduation from a school of professional nursing and one year of professional nursing experience; or an equivalent combination of training and experience.
